Making Your Appointment

Thank you for choosing us as your care provider.

When you call to make an appointment, you will be emailed a questionnaire regarding your pain / problem / injury. It is important to complete this before attending.

Your First Appointment

On the day of your appointment, please arrive 10 minutes early to ensure we have enough time to prepare any outstanding paperwork. If you have not already filled out the questionnaire mentioned above, please arrive 20-30 min before your appointment to complete this also.

We are mindful that your time is valuable and would like to avoid keeping you waiting. However, occasionally a patient may present that requires a little extra time, which may cause unforeseeable scheduling delays beyond our control. Where possible, we will advise you if we are running late. We apologise in advance if you are kept waiting.

During your first appointment, a thorough medical and diagnostic work-up may be required. If you have completed any tests (blood test results, MRI within 6-months, X-Ray, CT scan, etc), please bring these to your appointment so that Dr Keeley can review them during your appointment. We will usually try to access any imaging you may have had digitally, but we ask that you bring any hard copies of scans also, because there are times when we are unable to access the images (i.e: if a radiology centre server is down). When you call to make your appointment, Dr Keeley’s staff will ask you about any imaging you have already had done, and will advise you if you need any new scans prior to seeing him.

Online Registration

  • Once you have secured an appointment time and date, you will be requested to complete our Online Patient Registration Form prior to your appointment so that you don’t have to manually complete it on the day.
  • If you prefer to complete your questionnaire on the day in person, then please arrive earlier to allow time for your paperwork to be completed.

What to Bring

What to bring when you attend your appointment.

  • A current referral from your GP or Specialist
  • Medicare, DVA and Pension cards
  • Your Private Health Insurance details
  • Any tests or scans, including blood tests, ultrasounds, X-rays, MRIs or CTs you have had done
  • A list of all medications you are on (including prescription, vitamins, supplement or herbal medicines)
  • Any relevant correspondence from other specialists who may be involved in your care
  • List of any known allergies
  • Glasses (if you wear them)
  • A list of any questions you may wish to ask so you don’t forget on the day


You are welcome to bring a support person to your appointment if you wish to do so.

Post-Operative Appointment

If you undergo surgery, the necessary post-operative appointments will be scheduled.

These are important appointments to monitor your recovery and avoid complications.
